Never had a problem and good comment wearing a thong. One time I had to get a shot in the butt and when I pulled my shorts down the nurse commented, "That's convenient."

I do wear thong to the doctor if I need to.
One time I got some procedure and I was asked to get undress until my undies and the doctor and the nurse were looking at my with only my thong on, they didn’t say anything.

Yes I always wear a thong when I’m getting my annual skin cancer checkup, not much point covering your skin when it needs to be checked. My doctor raised his eyebrows and just got on with it, didn’t say a word. I actually think it makes his life easier. Next year I’ll wear a g string.

So just had my skin check and wore a thong. Totally professional and I think it helped because I noticed she checked a spot on my backside so who knows one day may save my life.

I don't have any other underwear, so it is a thong whenever I go see a doctor. No doctor has ever made a comment on my thong.

The only thongs I rarely wear are my wife’s and I’m really not comfortable wearing any of her underwear to the doctor, so for me it’s always one of my bikinis. My dr used to have patients put on shorts, but at my last physical I had a backless gown on and I know my underwear was seen. Nothing was said.

I’m currently doing a round of physical therapy for a hip issue. I go to my sessions wearing leggings and a thong. I need the range of motion both give me. Of course there is much hands on manipulation of my hip and leg, so it is noticeable that I don’t wear “traditional” underwear. Nothing is ever said as I would expect in a professional environment.

I wore one today, but I knew I would not have to drop my pants - it was just a simple 6-month follow-up. But if I had known, I would have needed to drop my shorts, I might would have worn regular briefs or boxer-briefs.

If I did that with my luck doc would be like “matter a fact let’s get your annual physical done, drop your pants”

The chances are good that he has seen WAY  more than one patient "going commando!" He won't "see anything he hasn't seen before" (including when he is showering alone! :P ), so go with what YOU enjoy!
